Seeing signs that hospital custodian Paul Buendia is ill, Shaun persuades a skeptical Melendez to order a battery of tests, which confirm he has pancreatic cancer. Because it has already advanced to stage 3, Melendez recommends against a risky surgery in favor of giving Paul a relatively comfortable year before he dies. When Shaun and Claire deliver the news, Paul wants to leave the decision up to his family. However, after they encourage him to go ahead with the operation, he takes a turn for the worse. Upon being admitted to the ER, sixteen-year-old Asha asks Dr. Lim to perform plastic surgery to repair the damage from a ritual female circumcision ordered by her parents. Though warned not to proceed without parental consent, Lim is prepared to accept the consequences, despite knowing Asha is using a fake ID that says she's 21. However, when Asha experiences complications that keep her in the hospital overnight, Lim has no choice but to tell her parents and Andrew. Meanwhile, as Dr. Glassman carefully scrutinizes every doctor being considered for his brain surgery, Shaun avoids spending time with his old friend Lea.
